Pt patient has NKDA no known drug allergies and comes into medical office at 1400 2:00 PM today. The patient is c/o complaining of a Pruritis itching from the medial aspect centre of his chest to the right lateral sided area. Upon observation, the patient is very Diaphoretic sweating and has some small papules raised bumps and erythema redness are noticed on the epidermal skin chest area extending out to the right, numerous Macules spots noted on the chest. There are also some papules bumps with small petechiae sites of bleeding on the distal far dorsal back end aspect of the RUE right upper extremity.
